Plus improve your overall health including sleep, stress, and activity in a supportive and fun environment.
Join us on a journey to improved wellness where you will work with a trained lifestyle coach and a support system for 24 one-hour sessions over the course of one year to gain the skills to make lasting changes.
Our new Lifestyle Change Program is proven to be effective in preventing diabetes and is being offered at no cost to our District residents of Fallbrook, Rainbow, Bonsall, and De Luz.

For residents of Fallbrook, Bonsall, Rainbow, and De Luz, our program is available at no cost to you. In order to ensure the sustainability of the program, we will collect insurance information and bill insurance providers on your behalf. Participants will not be required to make any direct payments. Your commitment to a healthier lifestyle is our focus, and we are dedicated to providing support without financial burden.

At Wellness Wednesday get your blood glucose screened, chat with Bianca our Lifestyle Coach, and attend a Cultivate Health Workshop all at the Community Health & Wellness Center, 1636 E. Mission Rd, Fallbrook.
