Self-Measured Blood Pressure Monitoring

Check. Track. Take Control of Your Health.

High blood pressure often has no symptoms - but knowing your numbers is one of the most important steps you can take for your health. Our Self-Measured Blood Pressure Monitoring Program makes it easy and convenient to monitor your blood pressure in a supportive, non-clinical setting.

High blood pressure affects nearly half of the U.S. adults, often without symptoms, leading to heart attack, stroke, and kidney disease.

How the Program Works

  • Check your Blood Pressure at the Community Health & Wellness Center: Stop in and use our easy-to-use blood pressure station during hours of operation; no appointment is needed.
  • Take a Monitor Home: Borrow a blood pressure monitor to use at home for up to two weeks.
  • Track Your Readings: Log your numbers over time to better understand your blood pressure patterns.
  • Learn What Your Numbers Mean: Educational resources are available to help you make sense of your readings and support heart-healthy habits.
